  • Must have computer skills and dexterity required for data entry and retrieval of patient information.
  • Must be proficient with Windows-style applications and keyboard.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Self-directed with the ability to multi-task.
  • Must be able to travel throughout and between facilities.
  • Must be able to work a flexible work schedule (e.g., more than 8 hours a day).Job Duties
    • Oversees a staff of Case Managers and Social Workers responsible for patient care coordination.
    • Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation.
    • Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards.
    • Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life.
    • Leads and directs the work of others.
