Job Description:
$10,000 sign on bonus available! This job improves outcomes by
reducing Heart Failure hospital readmissions and coordinating
episodes of care among patients recently admitted to the hospital
with a heart failure diagnosis. The Transitional Care RN Navigator
participates in identification of appropriate patients and
enrollment into the Transitional Care Program; conducts frequent
check-ins and provides education to the patient and family;
encourages patient and family engagement in self-care management;
assists the patient in navigating the healthcare system; and
promotes warm handovers to the next level of care by providing
timely, pertinent information in a standardized way. The clinic is
operational Monday to Friday (no weekends). This is an exempt
position.EducationRequired - Registered nurse diploma.Preferred -
Associate's or bachelor's degree in nursing.
Work ExperienceRequired - 2 years of nursing experience.
CertificationsRequired - Current registered nurse (RN) license in
state of practice.Basic Life Support (BLS) from the American Heart
Association.Preferred - Certification in clinical specialty
area.Knowledge Skills and Abilities (KSAs)

Physical and Environmental Demands
The physical essential functions of this job include (but are not
limited to) the following: Frequently exerting 10 to 20 pounds of
force to move objects; occasionally exerting up to 100 pounds of

incumbent works within a healthcare setting, there may be
occupational risk for exposure to hazardous medications or
hazardous waste within the environment through receipt, transport,
storage, preparation, dispensing, administration, cleaning and/or
disposal of contaminated waste. The risk level of exposure may
increase depending on the essential job duties of the role.
