Job Description Uptown Nursing Facility located in the Old Charity School of Nursing, seeking: Registered Nurse/Licensed Practical Nurse (LPN) Job Summary: The Floor Nurse is responsible for rendering nursing care in assigned area in terms of individualized resident needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse or Director of Nursing. Benefits for our Licensed Practical Nurses: Medical, Dental, and Vision Insurance PTO Sick Time 7 Paid Holidays Qualifications for LPNs: Graduation from an accredited school of nursing Current License (willing and able to provide documented evidence) Reliable Transportation Current CPR Card Duties of the Licensed Practical Nurse: Are as follows but not limited to; Provide nursing care only under the supervision of the DON, including the planning, implementation, documentation, and evaluation of the following: Supervise the CNAs to assure these tasks are performed properly. And assist with onboarding of new hires Providing for the emotional and physical comfort of residents such as Hygiene, ADLs, incontinent care, and environmental cleanliness. Observing, recording and reporting the condition of the resident including signs and symptoms which may be indicative of change in the resident---s condition to the DON such as lung sounds, vital signs, changes in pain status, skin integrity and document these changes while reporting them to the DON. Urgent changes, contact DON/ RN Supervisor and the attending Physician. Assisting with the rehabilitation of residents in accordance with their care plan. Medications Medications are properly stored and organized. Ensure that the orders on MAR matches physician---s order. Administers medications and topical treatments to each resident as prescribed andaccording to individual medication protocols. Documents immediately following med administration. Records any med errors and submits reports according to policy. Observes residents for possible side effects of medication administered and report to the RN Supervisor / DON. Follows proper drug disposal procedures regarding outdated medications, discontinued meds and controlled substances. Participates in controlled substances (narcotics) count at the beginning and end of each shift. Documents missed meds or over count and follows reporting procedures. Medical Appointments: Informs DON of emergency room visits, returns and hospitalizations. Provides current and accurate medical information as needed. Follows through with recommendations. Completes all documentation of medical visit outcomes. Privacy/Rights: Assure that all medical information is secured and kept private according to HIPPA regulations. Bathing, dressing, toileting, changing is all done in private. Residents participate in medical decisions according to their ability and/or have proper family/guardianship involvement. Each resident is entitled to his/her individualized health care plan and protocols. A resident---s confidential information should not be discussed in front of other people, including other resident---s or their family members. Documentation: Accurate report should be given at the beginning and ending of each shift Perform quality assurance rounds at a minimum of every 2 hours Document all falls, skin tears and an injury occurring on that shift. All documentation needs to be complete and accurate during each shift. The medication record should be up to date and recorded on the MARs. Maintain a pleasant and professional demeanor and work ethic at all times. All other duties as assigned Working Conditions/ Essential Physical Requirements: Intermittent physical activity including walking, standing, sitting, lifting and supporting patients. Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment. Incumbent will be required to work at any facility and be responsible for own transportation. Incumbent will be scheduled based on operational need. (rotate shifts, standby, AA, on-call) Incumbent may be required to work extended periods of time at a video display terminal (VDT). Incumbent may experience traumatic situations including psychiatric, dismembered and deceased patients. Incumbent will be required to use mechanical lifts, electronic scales, shower equipment, adjustable chairs/beds and various other equipment and supplies.
