The Health Coach will work as part of the Digital Medicine team to assist and inspire our patients to develop healthier habits, leading to better health outcomes in chronic disease management. The Health Coach will provide outreach to patients via telephone and gather required information, such as blood pressure readings and labs; to provide education, information, and encouragement to patients about available health interventions, including diet, fitness, behavioral and social connectivity programs; and to increase patient engagement, capability, and accountability. The Health Coach will promote wellness and help patients realize their personal best by partnering with them to facilitate behavior change. The Health Coach coordinates with pharmacists, physicians, nurses and other care providers; maintains documentation as required by law; and actively participates in special projects, as needed.
We are looking for people who are passionate about health and have excellent communication and interpersonal skills! Preferred backgrounds include nutrition, social work, social sciences, public health, athletics, and health education.
Required - High School diploma or equivalent
Preferred - Additional degrees, business training experience and/or certifications may be combined to meet minimum qualifications.
Required - 2 years experience working in field of applicant's training.
Required - Current certification required by clinical discipline if required (e.g., if a Pharmacy Technician, Pharmacy Technician Certificate issued by the Louisiana Board of Pharmacy)
Preferred - Certification from an accredited professional health coach training program
Knowledge Skills and Abilities (KSAs)
- Must have computer skills and dexterity required for data entry and retrieval of patient information. Must be proficient with Windows-style applications and keyboard.
- Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals.
- Excellent written and verbal communication skills
- Strong time management skills and self directed
- Collaborates with clinical staff to coordinate and gather pertinent information from patients to support clinical patient management.
- Provides education, information and encouragement to patients about available health interventions, including diet, fitness, behavioral and social connectivity programs.
- Communicates with patients to increase engagement, health capability and accountability in order to develop healthier habits and lead to better health outcomes.
- Maintains documentation as required by law
- Participate in special projects as needed
