This job description is intended to describe the general nature and level of the work being performed by people assigned to this work. POSITION SUMMARY:

The Transfer Center Director is responsible for successfully managing and operating a centralized patient transfer center. The Director provides the leadership and resources required to meet patient needs and financial objectives to support growth for LCMC Health’s Adult Hospital Market. This position must partner with leaders throughout LCMC Health to drive initiatives and services creating operational efficiencies. The Director is responsible for ensuring exceptional and efficient operational processes for the transfer center in accordance with the organization’s strategic and operational objectives. This includes communicating, directing, planning, organizing, and financial monitoring to facilitate the delivery of cost-effective, high quality health care services, in an ever changing and competitive healthcare environment.

JOB SPECIFICATIONS:

Education:

Minimum Required: Bachelor’s Degree required, graduate from an accredited School of Nursing, licensed to practice nursing in the State of Louisiana.

Preferred: Master’s highly preferred.

Experience:

Minimum Required: 5 years nursing experience; critical care and/or emergency room nursing experience preferred, of which must include 3 years in a supervisory capacity.

Minimum: Current, unencumbered RN License, in the state of Louisiana

Minimum Required:

Ability to effectively communicate with senior industry executives (internal and external to LCMC Health).

Ability and desire to integrate CQI/lean principles and tools into work processes.

The ability to set and maintain high standards of performance in a collegial environment, holding people accountable for achieving deliverable metrics and in doing so, one who encourages open dialogue and delegates authority and accountability comfortably.

Superior leadership skills, particularly in cultivating a high-performance leadership team, and in developing and maintaining excellent partnerships with physicians and staff. Must listen actively and accurately, encouraging input from others; provides clear directions.

Requires the emotional stability to function effectively in unpredictable situations; this role is subject to varying and unpredictable situations and/or irregular hours.

POSITION DUTIES:

Mission, Values and Standards of Ethical Conduct Oversees acceptance and transfer of emergent, critical and direct admit patients into Adult Hospital Market (AHM) facilities and, as applicable, works alongside the Children’s Director of Transport to facilitate transfer of emergent and critical patients to Children’s Hospital of New Orleans.

Monitor trends in patient volume, demand and resource utilization to appropriately initiate timely changes to meet operational goals.

Oversees software tools that collect clinical information including Bed Tracking software, Transfer Center database, call recording software and paging tools.

Promotes teamwork within and across the network by maintaining positive and productive intra-entity and intradepartmental relationships and utilizing effective problem-solving skills.

Adheres to the LCMC Health Standards of Ethical Conduct, displaying, as applicable, ethical behavior in the areas of business ethics, legal compliance, confidentiality, conflicts of interest, vendor relationships and the reporting of questionable activities.

Ensures that all Transfer Center and Bed Management personnel who have access to direct or indirect access to patient health information, treat that information with the strictest confidence.

Patient/Customer Engagement Collaborates with AHM facility leadership, physicians and other members of the health care team to assure alignment of the distinctive patient experience and provision of quality and safe care.

Provides operational oversight to areas of responsibility ensuring timely and safe patient transfers from referring institutions and other admission sources. Collaborates with clinical groups and physician groups to continually refine protocols to deliver and exceptional experience.

Assures patient safety by developing and implementing effective policies and processes that are compliant with the Joint Commission and regulatory requirements.

Works with local and regional ground and air ambulance services to assure safe and timely transfer of patients to LCMC Health.

Provides expert guidance, consultation and leadership to staff to attain optimal results; organizes and provides orientation and training for new employees and performs ongoing performance management for staff.

Performs quality assurance audits for Transfer Center and Bed Management operations and clinical medical care during patient transports.

Develops and implements annual outreach strategies for the purpose of identifying and responding to customer needs and, as applicable, complaints.

Physician/Employee Engagement Provides support and assures that employees are performing their work consistent with organizational expectations and policies and procedures.

Communicates, facilitates, translates, establishes boundaries and celebrates with team members to achieve exceptional employee engagement.

Assesses and assures selection, coaching, development and alignment of the team members work with achieving departmental plans based on organizational expectations.

Financial/Growth/Development Understand finance principles, budgets, reimbursement principles, capital expenditures, productivity measures and ensure department(s) meet financial expectations.

Develops and continually monitors key priorities and goals, collects and analyzes data and presents results to both staff and decision makers.

Continually measures progress towards key goals, collaborating with strategy and business development, marketing, and physician specialty groups to increase number of patient referrals.
