Responsibilities:

* Responsible for leading and facilitating the quality operations of Tulane Health System's Main and Lakeside Campuses.

* Responsibility includes Infection Control, Risk Management/Patient Safety, Accreditation/Regulatory Compliance, Patient Safety, Data Analytics and Reporting, and Quality Improvement.

* Responsible for leading, managing, and coaching direct reports.

* Supports the System VP Quality and Patient Safety in prioritizing system-wide quality initiatives.

* Leads, manages, and coaches quality staff.

* Establishes regular communications to leadership and staff.

* Maintains Accreditation compliance and readiness.

* Identifies patterns and trends identified from tracers, patient safety events and registry data.

* Oversees all SEA/RCAs including improvement plans.

* Serves as contact person for all on site surveys i.e. TJC, CMS, LDH, OSHA, etc.

* Communicates and/or updates Leadership about changes in regulatory standards, quality improvement techniques, lessons learned from patient safety data (including RCAs and FMEA) and patient, provider and employee satisfaction results and improvement plans.

* Collaborates with Lakeview Campus to evaluate and create the annual Quality/Patient Safety Plan and obtain approval through the appropriate committees.

* Responsible for internal and external reporting based on federal, state, and other regulatory requirements including sentinel events and notifying division and system leadership of all reporting.

* Accountable for making improvement recommendations on all root cause analysis results to ensure consistencies with processes and policies.

* Oversees action plans are being monitored to ensure sustainability.

* Ensures accurate Infection Prevention & Control surveillance data collection, validation and reporting.

* Participates with Infection Prevention & Control in developing the annual infection control risk assessment and plan.

* Active member of the Infection Prevention & Control Committee.

* Oversees Regulatory, accreditation and Certification maintenance through continuous survey readiness.

* Aggregates results from tracers and share with entity leadership and system coordinator.

* Serves as system coordinator for all on site surveys.

* Develops responses to survey deficiencies and submit responses to the appropriate organization (TJC, CMS, LDH, OSHA, etc.).

* Oversees Quality Data Reporting and Analytics requirements.

* Timely collaboration with system VP Quality and Patient Safety on any validation data sent to external sources i.e. TJC, LDH, CMS, NHSN, and other registries as applicable.

* Timely internal reports to system VP Quality and Patient Safety.

* Oversees medical staff mortality and complication reviews and referrals for peer review.

* Coordinates Quality Improvement Committee (QIC) of the Board of Governors.

* Develops and implements process improvement plans and assure participation in coordinated efforts across the system.

* Leads efforts to implement High Reliability in processes and culture.

* Advances a patient safety program that promotes a culture of safety and the elimination of avoidable harm.

* Prevents future harm by initiating and overseeing proactive evaluation and redesign of systems to improve care processes (e.g. forcing functions, checklists, error causation thinking, human factors, applied informatics, culture).

* Improves consistent delivery of evidence-based care and reduction in preventable harm by focusing on reliability and applying the principles of reliable design.

* Utilizes alerts and best practices (e.g. Sentinel Event Alerts) to perform gap assessments and implement strong actions that will alleviate identified gaps.

* Effectively report, investigate, and analyze patient safety incidents, medical errors and potential risks in the facility.

* Facilitates thorough and credible serious event analysis that result in strong sustainable improvement strategies.

* Facilitates thorough and credible failure mode effect analysis to identify and mitigate unintended adverse patient outcomes and evaluate effectiveness of process changes.

* Coordinates disclosure of serious events to patients and/or families in accordance with organizational policy and regulations.

* Assures timely reporting of Patient Safety Work Product (PSWP) to the Patient Safety Organization (PSO).

* Actively participates in PSO learning collaboratives.

* Ensures implementation of best practices, alerts, and updates to drive patient safety improvement.

* Champion completion of Culture of Safety Survey.

* Facilitates thorough and credible review of events that address both system and individual accountability

* Provides ongoing education to leaders, clinicians and staff on the science of safety (high reliability, effective communication, sustaining awareness/alertness) and patient safety initiatives

* Provides analysis and identifying trends from reports to track progress of improvement strategies. Spread and sustain improvement.

* Presents informative and actionable patient safety reports to appropriate committees to include high level presentations to Leadership, Medical Executive Committee and Governing Board.

* Oversees Research Study Approval Process

What Qualifications You Will Need

* Appropriate licensure for healthcare related degree. CPHQ and CPPS Certification required, CJCP preferred.

* Master's degree in a clinical discipline required.

* Clinical background required.

* Minimum of 5 years progressive hospital clinical leadership experience.

* Experience in coordinating regulatory agency survey activities including TJC, CMS, LDH and other reviews related to hospitals and outpatient clinics required.

* Experience in patient safety, risk, infection control and quality required.

* Healthcare clinical experience must be recent (within the last 2 years) and within a clinical setting such as a hospital, ambulatory surgery center, etc.
