Available! This job provides individualized, direct nursing care of
assigned patients under the direction of a registered nurse,
mid-level provider or physician. Assists in data collection and
implementation of the delivery of health care services, and assists
other members of the healthcare team in providing the highest
quality personalized patient care. Maintains a positive work
relationship with the health care team, and exhibits the skills to
provide care and service with individuals of all assigned age

Required - Graduate of an accredited School of Practical
Required - 2 years LPN experience or one year of LPN
experience with one year supervisory experience.
Preferred - Experience in ambulatory care.
Required - Current LPN License in the state of practice
Basic Life Support (BLS) from the American Heart Association
Knowledge Skills and Abilities (KSAs)
- Must have computer skills and dexterity required for data entry
and retrieval of patient information. Must be proficient with
Windows-style applications and keyboard.
- Effective verbal and written communication skills and the
ability to present information clearly and professionally to
varying levels of individuals throughout the patient care
