# #The Care Coordinator Assistant (CCA) will be responsible for assisting the Case Managers and Social Workers with discharge arrangements/services, serving patient IMMs, preparation and throughput to nursing homes, organizing and updating applicable CM spreadsheets, and other duties as identified to assist the department in meeting the care coordination needs of our patients during and sometimes post discharge. When needed, the CCA will provide clinical information to applicable insurance providers in order to obtain necessary authorization. The CCA will be assisting with inputing data into the financial system as well as some data entry into the electronic medical record. # Job Specifications - Education and Experience High school graduate.
