HIRING BONUS-This position also includes a $500 sign on bonus!
$200 at hire, $100 after 60 days, $100 after 120 days, $100 at 6
months. Must be employed at time of pay out and if you leave prior
to the full payout, the signing bonus will be deducted from your

Easterseals Louisiana (ESL) is a nonprofit organization whose

8,600 individuals (whom we refer to as Participants) with a variety
of disabilities. A leading provider of supports to individuals with
disabilities, ESL has physical locations in 8 regions of Louisiana
and our array of programs include; support coordination (case
management) for children, adults, and aging participants, a wide
range of behavioral health and housing supports, early intervention
programs for infants and toddlers, and The Chartwell Center for
children and young adults with Autism in New Orleans.

Our Support Coordinators (case managers) who serve the OCDD (Office
of Citizens with Developmental Disabilities) population, serve a
caseload of about 35-45 participants. These participants live in
their homes and communities and receive support (which can be
physical, medical, educational, or vocational) to assist them to
remain in their desired settings. The primary role of the Support
Coordinator is to plan, organize, implement, and monitor the
in-home and community based services and support the participant
receives.

All new hires complete an initial training program of about 6
weeks. During this time you will learn how to plan, schedule, and
coordinate services to meet our participants' needs. Learning about
the OCDD service system and requirements, making monthly calls,
quarterly visits, and annual CPOCs (Comprehensive Plan of Care) are
all responsibilities our SCs have, along with maintaining any
pertinent medical and health information for your assigned
participants. Support Coordinators must be agile and responsive in
their ability to meet the changing needs of participants and
service provision requirements. Support Coordinators are also
responsible for the support of participants throughout emergent
situations and all public emergencies including weather related
evacuations. While this position is a hybrid office and remote
work, for the first 90-day you will be required to be in the office
full time.

Requirements
--
This role requires providing supports in a home and community-based
setting, so it is vital that you have a reliable form of
transportation to visit participants in their homes. Support
Coordinators must be able to use web-based applications, such as
the Google Workspace applications, and Microsoft Word programs.

A bachelor's degree in social or human services is required for
this position. If you have a bachelor's degree in another field you
must have at least 16 hours of social or human services within your
bachelor's degree. Successfully passing a background check and drug
screening are required for this position.

Qualifications:
A Bachelor's or Master's Degree in Social Work from a program
accredited by the Council on Social Work Education-- or
A Bachelor's or Master's Degree in Nursing (RN) currently licensed
in Louisiana (one year of paid experience as a licensed RN will
substitute for the degree)-- or
A Bachelor's or Master's Degree in a Human Service related field
(i.e. psychology, education, counseling, social services,
sociology, philosophy, family and participant sciences, criminal
justice, rehabilitation services, substance abuse treatment,
gerontology, and vocational rehabilitation) or
A Bachelor's Degree in Liberal Arts or General Studies with a
concentration of at least 16 hours in one of the following fields
(i.e. psychology, education, counseling, social services,
sociology, philosophy, family and participant sciences, criminal
justice, rehabilitation services, substance abuse treatment,
gerontology, and or vocational rehabilitation)
Two (2) years of paid post degree experience providing support
coordination services preferred.
Reliable transportation so that you may make visits to the field to
conduct participant visits. These visits are generally held in the
home of the participant. You may anticipate about 50% or more of
your time will be in the field on home visits.
The employee must be able to operate a computer and use web-based
applications.
Easterseals Louisiana is a Drug-Free Workplace, pre-employment and

Will be required to pass Easterseals Louisiana's background check
and driver's license check

It is important to support coordination that employees understand

and the waiver program group includes participation in office
schedules and good attendance.

Starting Salary:$33,500 Commensurate on experience
