This job assists with the medication reconciliation process by ensuring an accurate and complete home medication list is documented for admitted patients. Communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain an accurate and complete medication history for patients. Identify discrepancies or problems with medication history information and notify a pharmacist to seek clarification or resolution. Ensure capture of prescription benefit information and document the home medication list in the electronic health record (EHR).This job description is a summary of the primary duties and responsibilities of the job and position. • Must have computer skills and dexterity required for data entry and retrieval of information.
• Effective verbal and written communication skills and the ability to present information clearly and professionally.
• Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
• Strong interpersonal skills.Job Duties
  • Accurately obtains and clarifies patients' home/chronic medication list by contacting community pharmacies, physician offices and/or extended care facilities, interviewing patient and/or family members to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and complimentary & alternative medications in a timely manner.
  • Utilizes the electronic health record to input patients' home/chronic medication lists for the purposes of medication reconciliation under the supervision of a pharmacist.
  • Communicates discrepancies identified via the medication history process with the pharmacist and makes clarifications with the patient and prescribers as needed.
  • Accurately collects patients' prescription benefit information.
  • Appropriately directs, triages or communicates to the appropriate caregivers or staff all questions or inquiries from patient or families that are outside the scope of taking the medication history.
  • Provides high quality customer service to patients, family members, and care team members.
  • Ensures capture of appropriate patient information necessary to generate safe and effective medication orders (i.e. allergies, height, weight).
  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
